Prolia (denosumab) is a rank ligand inhibitor (biologic injection) made by Amgen. It is commonly prescribed for: Postmenopausal osteoporosis in women at high risk for fracture, Osteoporosis in men at high risk for fracture, Glucocorticoid-induced osteoporosis in men and women at high risk for fracture, Bone loss in men receiving androgen deprivation therapy for nonmetastatic prostate cancer, Bone loss in women receiving adjuvant aromatase inhibitor therapy for nonmetastatic breast cancer.

If your plan or its pharmacy benefit manager runs a copay maximizer (it requires you to enroll in this card as a condition of waiving your cost sharing), Amgen may change the card's benefit amount at its sole discretion. Amgen also publishes no dollar figure for the Maximum Program Benefit or the Patient Total Program Benefit, and says the Patient Total Program Benefit may be lower than the Maximum Program Benefit depending on your plan.
